DOT 5-Panel Urine Drug Test

PhlebCARE provides DOT 5-panel urine drug testing for employers and safety-sensitive employees who are subject to Department of Transportation drug testing requirements. This service may be used for pre-employment, random, post-accident, reasonable suspicion, return-to-duty, or follow-up testing when requested by the employer or Designated Employer Representative.

DOT drug testing follows federal collection procedures under 49 CFR Part 40, which governs workplace drug and alcohol testing for federally regulated transportation industries. The DOT 5-panel drug test screens for marijuana, cocaine, amphetamines, opioids, and PCP.

Important Notes:

This service is for DOT drug testing only. Breath alcohol testing is not currently included.

For return-to-duty and follow-up testing, the employer or DER must provide the proper testing instructions. DOT rules require direct observation for return-to-duty and follow-up drug test collections.
